SENIOR MOMENT UPDATE

• October 12th at 1:30 at the FDL Public Library located at 32 Sheboygan Street.

OPEN ENROLLMENT: Open enrollment for Medicare Part D and Advantage Plans starts October 15th and runs through December 7th. Beneficiaries are starting to receive their 2012 information. If you would like to make a change for 2012 please contact Julie at 929-3521. Also, new to 2012 if you have an Advantage Plan with a 5 or less star rating you can switch to a 5 star plan at any time throughout the year with no penalty.

• Plans that are no longer available for 2012: Advantage Star, Bravo Rx, Cigna Plan 2 and Sterling Rx.

• Anthem BC/BS is new in 2012
• Monthly premiums ranging from $0 to $135.00 per month (co-payments and co-insurance applicable)

FLU SHOTS UNDER MEDICARE – It’s that time of year again for your flu shot, clinics are available throughout FDL County as well as at the FDL County Health Dept. located 3rd floor of the City/County Building. Bring your Medicare Card or Advantage Card for billing purposes. Medicare covers 100% of the flu shot.

Wisconsin’s New Voter Photo ID Law
Gov. Scott Walker signed a bill on Wednesday, May 25 that will require voters in the state to show photo identification when they go to the polls. The law will requires poll workers to immediately begin asking voters to show state-issued IDs before they vote. But voters won't be required to actually produce the IDs starting with the 2012 February primary. In the meantime, voters who come to the polls without IDs will be handed a flyer outlining the new requirement.

An acceptable ID is not required to have a current address, examples of an acceptable ID is listed below:
• A Wisconsin DOT-issued driver’s license, even if driving privileges are revoked or suspended.
• A Wisconsin DOT-issued identification card
• Military ID card issued by a U.S. uniformed service
• U.S. passport
• Certificate of naturalization issued no earlier than 2 years before the date of an election at which it is presented
• Unexpired driving receipt issue by DOT
• Unexpired ID cards issued by DOT
• ID card issued by federally recognized Indian tribe in Wisconsin
• Unexpired ID card issued by a Wisconsin accredited university of college

For those that do not have an ID there is an option in FDL County to obtain photo identification.
• Department of Motor Vehicles
o Any age, must appear in person
o Free, if indicate it is for voting purposes
o Must provide certified copy of birth certificate, passport, expired driver’s license, social security card with signature and proof of residency.

Flu Shots and Tetanus shots. It’s that time of year again for the flu shot. Clinics are being set up by the Health Dept. for FDL County, check your local papers for dates and times. Medicare completely covers the flu shot. Unfortunately if you have a tetanus shot administered by your physician it is not covered and can cost you up to $104.00. You can receive that same tetanus shot at the FDL County Health Dept. for free.
