Preventative Benefits under Medicare

Welcome to Medicare Exam
What happens during the visit:
• Record your medical history
• Check your height, weight and blood pressure
• Calculate your body mass index
• Give you a simple vision test
• Depending on general health other tests maybe ordered
Caution
• Must be schedule within first 12 months of effective date of Part B
• NOT a routine physical/checkup
• Does not include any lab tests
• Exam must contain 7 Medicare-specific components to be covered
• Beneficiary must specifically call and schedule WTME.


Annual Wellness Visit
Starting January 1, 2011, if you’ve had Part B for longer than 12 months you can get a yearly “wellness” visit to develop or update a personalized preventive plan based on your current health and risk factors.
What happens during the visit:
• Review of medical and family history
• A list of current providers and prescription
• Height, weight, blood pressure and other routine measurements
• A screening schedule for appropriate preventative services
• A list of risk factors and treatment options for you
Covered once every 12 months
Caution
• NOT a routine physical/checkup
• Does not include any lab tests
• Visit is meant to promote and develop plan for preventative services
• Procedures scheduled as a result of this visit may have copays, coinsurance and deductibles
• Patients that bring up ailments during the wellness visit may be charged for a separate office visit

Project “Heat’s On” - On Saturday, October 1, 2011 beginning at 8:30 a.m. The “Heat’s On” community assistance program is working to improve the health and welfare of our communities by ensuring their heating systems are safe and reliable for the upcoming winter. The Project “Heats On” volunteers will come into your home to inspect your heating system, change furnace filters and inspect smoke alarms and put new batteries in them. This is will be an annual program for the senior citizen homeowners in Fond du Lac County.
To Qualify:
 Need to be 60 years of age or older
 Homeowner and have a current property tax bill
 Reside in City of Fond du Lac or Village of North Fond du Lac
 Be within the income guidelines:
• Single $1,588 monthly/$19,056 annually
• Couple $2,145 monthly/$25,740 annually

MARK YOUR CALENDARS!

PLEASE NOTE – Starting in calendar year 2011, there will be a change in the dates of the Open Enrollment Period for Medicare Part D and Medicare Advantage Plans. The new dates for changing your Medicare Part D and Medicare Advantage plans will be October 15 – December 7, 2011 for plans to be effective January 1, 2012. You will not be able to change plans for the next year after December 7. Plans will send beneficiaries notices of change for 2012 around the first part of September. If you do not like the changes for the coming year, you have this one time opportunity to check out other plans that maybe more cost effective and meet your needs. VOLUNTEER’S NEEDED – The Fond du Lac Nutrition program is in need of volunteers for the meal program. We serve Monday through Friday and the times are from 10:30 to 1:00 p.m. Volunteers are provided a free meal for the day they volunteer as well as leftovers to take home.
