SENIOR MOMENT UPDATE

Seniors Eyecare Program
Senior eyecare program ensures that every senior has access to medical eye care and promotes annual, dilated eye exams. People eligible for a referral through the program receive a comprehensive medical eye exam and up to one year of treatment at no out-of-pocket expense. Call 1-800-222-3937.
Who is eligible: Age 65 or older
US Citizen or legal residence
Have not seen ophthalmologist in 3 or more years
Do not have eye care insurance through HMO or VA

Hear Now (Hearing Aides)
The Hearing Foundation’s US program provides hearing aids and a hearing test report to those who are legal residents of the US, meet the income and asset guidelines, complete the application process and are approved for assistance. There is a non-refundable deposit on the hearing aids that is $100 for one and $200 for two.
Income Guidelines
Family of 1 - $12,000
Family of 2 - $16,000
*Assets are also considered when reviewing the application.

Wisconsin Well Woman Medicaid
Wisconsin Well Woman Medicaid provides full Medicaid benefits to underinsured or uninsured women under age 65 who are screened through the Wisconsin Well Woman Program and are in need of treatment for breast or cervical cancer. To be eligible you must meet the following criteria:
Meet income requirement of single - $22,450 or couple $30,300
Be at least 35 but under 65 years of age
Be a resident of Wisconsin and a citizen or qualified alien
Provide social security number
Not be eligible for private or public health care coverage
Have been screened for breast or cervical cancer by the program
Require treatment for breast or cervical cancer, or a precancerous condition of the cervix.

SENIOR MOMENT UPDATE

When I’m 65 Presentation: On February 23, 2010 at 1:00 p.m. at the FDL Public Library. Julie Hilbert will be presenting “When I’m 65” which is an informational seminar on Medicare, Insurance, working past retirement and Medicare Part D. To reserve your spot please contact Julie at 929-3521.

Medicare Part D Open Enrollment for those who are already on Medicare has ended on December 31, 2009. If someone is interested in a Medicare Advantage plan or Medicare replacement policy they have an extended open enrollment through March 31, 2010. For more detailed information on these options please contact Julie at 929-3521.

Assistance w/Medicare Costs
QMB - The monthly income limit for the QMB program is $902.50 for an individual and $1,214.17 for a couple. The QMB program pays Medicare’s premiums, deductibles, and coinsurance amounts.
SLMB - The monthly income limit for the SLMB program is $1,083 for an individual and $1,457 for a couple. The SLMB program pays Medicare’s monthly Part B premium of $93.50 per month per eligible individual.
SLMB + - The monthly income limit for the SLMB+ program is $1,218.38 for an individual and $1,639.13 for a couple. The SLMB+ programs help pay Medicare’s monthly Part B premium of $93.50 per month per eligible individual.
**Asset limit $6,600 single and $9,910 for couple.
