Lifeline/Telecare
Provides elderly persons who live alone with emergency communications system that attaches to home telephone. A device is worn around neck in case of falls or other emergencies.

PERS-2400 Emergency Reporting System – This is an emergency reporting system so that people can live independently, freely and confidently in their own home with peace of mind. The unit is only $23.95 per month. The company, Advance is out of Madison, WI.

Yard Work and Snow Removal
The Department of Senior Services has information on individuals that do yard work for seniors in the community. Prices are based on the size of the yard and the landscape that is involved. Please call Julie at 920-929-3521 for more information.

Senior Services Transportation
Escort (Car) – For frail people of all ages. Assist passengers in building and will leave county for medical appointments only. Cost is $2 within city limits and $5 outside FDL County. Must have physician sign authorization for specialized transport. Call one month in advance 929-3110

Handi-van (Lift-Equipped) – For mobility challenged people of all ages. Door to door services. $2.50 charge one way within city limits. $9.00 charge one way, outside city limits. Will not leave county. Call 2 weeks in advance 929-3110.

Elderly (regular sized van) – People 60+. Must be able to step up on van. Curb to curb services. Within City limits only. Donation of $2.50 per way for medical, employment, nutrition rides, personal business, education/training and social or recreational rides. Call 2 weeks advance 929-3936.

Seniors Eyecare Program
Senior eyecare program ensures that every senior has access to medical eye care and promotes annual, dilated eye exams. People eligible for a referral through the program receive a comprehensive medical eye exam and up to one year of treatment at no out-of-pocket expense. Call 1-800-222-3937.
Who is eligible: Age 65 or older
US Citizen or legal residence
Have not seen ophthalmologist in 3 or more years
Do not have eye care insurance through HMO or VA

Shopko Vision Senior Discounts – If you are an AARP member you can receive discounts of 30% on eyeglasses (frames and lenses). They also offer senior discounts of 15% every Wednesday. Please visit or call the local FDL Shopko for further details.

Hear Now (Hearing Aides)
The Hearing Foundation’s US program provides hearing aids and a hearing test report to those who are legal residents of the US, meet the income and asset guidelines, complete the application process and are approved for assistance. There is a non-refundable deposit on the hearing aids that is $100 for one and $200 for two. 1-800-328-8602
