Beware of the Green postcards – People have been receiving green post cards that state “Important Benefit Supplement Information for seniors”. If you receive such a card please do not put down your information and send it back. If you send it back, you will be contacted by a sales representative who will attempt to sell you something you may not want or need. If you have questions regarding insurance, long-term care and death benefits please contact your local benefit specialist, Julie Hilbert at 929-3521.NEW DISCOUNT PRESCRIPTION CARD – Family Wise is a free discount card where you can save up to 30% on your prescriptions. All of the FDL County pharmacies accept this card and all members of a family household can use it. Almost all prescriptions are covered under this card. To get your card please visit www.familywize.org or call 1-800-222-2818.Free Minutes and Cell phone for people on Medicaid – Contact 1-877-870-9444 or www.reachoutwireless.com to receive 50 cell phone minutes, phone and shipping free. You cannot have Wisconsin Lifeline and Linkup program if you would like this program. To replenish minutes it’s as follows:25 minutes = $5.00120 minutes = $10.00225 minutes = $20.00350 minutes = $30.00Grant money available for victims of ’08 floodFond du Lac County has been awarded $500,000 through the Community Development Block Grant - Emergency Assistance Program (CDBG-EAP) that will assist households with June 2008 flood damage. Applications are accepted on a first-come, first-serve basis. There is no cost to fill out an application and all information submitted is strictly confidential. These funds are for residents of Fond du Lac County, the funds need to be spent by Dec. 31, 2009, so don’t wait.Eligibility:Cannot be resident of the City of Fond du LacMust be primary residenceAnnual income restrictions: Single $36,750 – Couple $42,000Funds used for eligible disaster related expensesFor more information or to obtain an application, contact Stacy Griswold at the Housing Program office at 1-800-552-6330
